Case Management

Case management is an opportunity to review needs and resources, then link a person to a full range of appropriate services, using all available funding sources. Services are monitored over an extended period of time and modified as needed. The purpose of case management is to ensure that older Kansans are made aware of the option of community-based long-term care services (as opposed to nursing home care) and to facilitate the coordination of those services on their behalf.
